Which rules does this use?

The official Ukrainian transliteration adopted by Cabinet of Ministers Resolution No. 55 of 27 January 2010. It is used for passports, driving licenses and road signs.

Why is Є sometimes "Ye" and sometimes "ie"?

The rules depend on the position in the word. At the start of a word Є, Ї, Й, Ю and Я become Ye, Yi, Y, Yu and Ya. Anywhere else they become ie, i, i, iu and ia. So Юлія becomes Yuliia.

What happens to the soft sign and apostrophe?

Both are left out, as the rules require. The combination зг is written as zgh (Згорани becomes Zghorany) so it is not confused with the letter ж.
